2 <br />1. Truthfully complete an application to participate in the Program. <br />2. Sign a waiver of liability and authorization for release of information <br />forms. <br />3. Return the application forms to the City Administrator or Fire Chief (or <br />designee). <br />4. The completed application shall be forwarded to the Department Head for <br />processing and review to determine the person’s eligibility to participate. <br />a. If the person is eligible for the Program, the Department Head or a designee shall <br />notify the applicant that they are eligible to participate. <br />o The person making the determination of eligibility shall note their name and <br />date on the application. <br />o The person shall be assigned specific hours to participate. These hours shall <br />be noted on the application along with the name of the assigned officer and <br />the date the assignment was made. <br />o The assigned individual affected by the hours shall be given a copy of the <br />approved application, signed waiver and shall be informed of the <br />arrangements that have been made. <br />b. If the person is ineligible to participate in the Program, the City Administrator or Fire <br />Chief (or designee) shall notify the applicant that they are not eligible. <br />o The reasons for ineligibility shall be noted on the application along with the <br />name of the person making the determination. <br />o The date the person was notified of the ineligibility shall be noted on the <br />application. <br />c. All application shall be maintained in an administrative file for a period of three <br />years <br />RULES <br /> <br />a. Participants do not have the power to make any decisions on the behalf of the City <br />of Hugo while involved in a ride-along. Individuals participating in the program with <br />the Fire Department or Police Department have no police power and shall not take <br />any law enforcement action unless they are directed to do so by an officer of the <br />Department. <br />b. Participants shall respect and maintain the confidentiality of the City’s information <br />and records. <br />c. Participants shall not use City communications equipment except in extreme <br />emergencies. <br />d. Participants shall not use City computers except as authorized by the City <br />Administrator.
