5 -2.01 5 -2.03 <br /> 5 -2.01 Licensing Procedure. <br /> A. Application and Issuance. Application for <br /> contractor's licenses must be on forms provided by the <br /> city, accompanied by the appropriate fee as stipulated <br /> in Section 5 -5.01 of this code, and any special <br /> requirements set forth for specific contractors. Such <br /> application shall be submitted to the city council for <br /> consideration. <br /> B. Revocation. Every license may be revoked by the <br /> city council for just cause providing the license <br /> holder has been given reasonable notice and a hearing <br /> is held. <br /> C. Reapplication following Denial. In the event an <br /> individual is denied a license, that individual may not <br /> reapply for a license until six (6) months have passed <br /> from the date of denial. <br /> 5 -2.03 Regulation of Specific Contractors. <br /> A. Refuse Haulers <br /> 1. Licensing Requirements <br /> a. Must be capable of at least weekly <br /> collection of rubbish and other waste <br /> materials including seasonal collection of <br /> leaves, grass clippings and brush. <br /> b. Provide for periodic removal of excess <br /> refuse. <br /> c. Must offer a minimum of three different <br /> volume based rates, including 32 gallons per <br /> week, 64 gallons per week, and greater than <br /> 64 gallons per week. <br /> d. File with the city clerk a schedule of <br /> proposed rates to be charged during the <br /> licensing period. Every licensee shall <br /> provide 14 days written notification to the <br /> city and the licensee's customers of any <br /> change in rates to be implemented during the <br /> licensing period. <br /> e. Refuse collection in R -2 and R -1 zones <br /> must be provided on Fridays only. Exception: <br /> 6 <br />
