a time consistent with other employees and in a manner subject to the approval of the City <br />Council of Little Canada and in compliance with City's comparable worth plan. City agrees <br />to provide a performance review of Heller on an annual basis. <br />4. DEFERRED COMPENSATION. City shall allow Heller to enroll in the ICMA-RC <br />deferred compensation plan with contributions to this plan to be the sole responsibility of <br />Heller, <br />5. PAID TIME OFF (PTO). Upon the commencement of employment, Heller shall be <br />provided with six (6) days of PTO accrual for use by Heller, subject to approval of the <br />specific dates by the City Administrator, PTO shall be accrued by Heller as outlined in <br />Section 14 of the City's Personnel Policy based upon Heller's initial employment date of <br />November 2, 2015. Heller shall also be eligible for one week's additional PTO on January 1, <br />2017 and each year thereafter subject to satisfactory performance. <br />6. FIOLIDAYS. City will provide Heller the same standard holidays as enjoyed by other <br />employees as set forth in Section 13 of City's Personnel Policy. <br />7, INSURANCE. City will pay the cost of hospital and medical insurance for Heller and <br />dependents in the same manner provided for other employees pursuant to Section 12 of City's <br />Personnel Policy. Heller may also participate in the "Opt -Out" program provided for in <br />Section 12 of the City's Personnel Policy on the same basis as other employees. City will <br />provide Heller the same group life insurance coverage that is provided to all other City <br />employees. City further agrees to continue to enroll Beller in long-term disability coverage in <br />a manner consistent with other employees. The cost of long-term disability coverage is <br />Heller's responsibility. <br />2 <br />
