Appendix G <br /> <br />CRYOGENIS FLUIDS-WEIGHTS AND VOLUME EQUIVALENTS <br />This appendix is used to convert from liquid to gas for cryogenic fluids. <br />At this time the only building that this would refer to is St. Jude Medical. <br /> <br /> <br />Appendix H <br /> <br />HAZARDOUS MATERIALS MANAGEMENT PLAN (HMMP) AND HAZARDOUS <br />MATERIALS INVENTORY STATEMENT (HMIS) INSTRUCTIONS <br />Appendix H requires site plans, floor plans and product inventory. Facilities which have <br />prepared, filed and submitted a Tier II Inventory Report required by the U.S. Environmental <br />Protection Agency (USEPA) or required by a state which has secured USEPA approval for a <br />similar form shall be deemed to have complied with this section. <br /> <br />EMERGENCY PLAN <br />1. Emergency Notification. <br />2. Where OSHA or state regulations require a facility to have either an Emergency Action Plan <br />(EAP) or an Emergency Response Plan (ERP), the EAP or ERP shall be included as part of the <br />HMMP. <br /> <br />This hazardous material management plan and hazardous materials inventory statement would be <br />required for business with reportable amounts of hazardous materials. <br /> <br />A site plan on 81/2 by 11 inch paper showing the locations of all buildings, structures, outdoor <br />chemical control or storage and use areas, parking lots, internal roads, storm and sanitary sewers, <br />wells and adjacent property uses. <br /> <br />Provide a floor plan of each building identified on the site plan as containing hazardous materials <br />on 81/2-inch by 11 -inch (215 mm by 279 mm) paper, identifying the northern direction, and <br />showing the location of each storage and use area. <br /> <br />This is valuable information for respond emergency personnel for life saving and fire- fighting <br />activities. <br /> <br />REFERENCED STANDARDS <br />ICC IBC—12 International Building Code <br />ICC IFC—12 International Fire Code <br /> <br /> <br />Appendix I <br /> <br />FIRE PROTECTION SYSTEMS—NONCOMPLIANT CONDITIONS <br />This appendix is intended to identify conditions that can occur when fire protection systems are <br />not properly maintained or components have been damaged.
