COVID-19 Preparedness Plan <br /> <br /> <br />Page 8 <br />Situation Are you feeling ill or <br />fall above thresholds <br />from THRESHOLD <br />CHART below <br />What should you do? <br />Employee has been advised <br />by a health care provider to <br />self-quarantine. <br />Yes Stay Home – Follow your health care provider’s advice – you <br />will be paid using Federal Emergency Paid Sick Leave (for up to <br />80 hours) and are eligible for City Emergency Pandemic Leave. <br />Use the CDC Guidelines below to return to work. You may be <br />asked to provide a doctor’s note to return to work. <br />No Stay Home – Follow your health care provider’s advice. If you <br />are able to work from home the expectation is that you will <br />work. If it is not possible to work from home, you will be paid <br />using Federal Emergency Paid Sick Leave (for up to 80 hours) <br />and are eligible for City Emergency Pandemic Leave. Connect <br />with your Supervisor/Manager to have a work plan. <br />Use the CDC Guidelines below to return to work. You may be <br />asked to provide a doctor’s note to return to work. <br />Employee has been in close <br />contact with someone who <br />has tested positive for COVID- <br />19. <br />Yes Stay Home – Follow your health care provider’s advice. You <br />may be eligible for Federal Emergency Paid Sick Leave and City <br />Pandemic Leave. Use the CDC Guidelines below to return to <br />work. <br />No Stay Home for 14 days and practice social distancing and <br />monitor symptoms - Connect with your Supervisor/Manager – <br />to determine if you are able to work remotely if not already <br />doing so. If you are able to work from home the expectation is <br />that you will work. If not, you may be eligible for Federal <br />Emergency Paid Sick Leave and City Pandemic Leave. <br />Use the Guidelines for Exposure below to return to work. If <br />you become symptomatic, notify your Supervisor. <br />
