Section 12 — Insurance Benefits (Page 16) — <br />Health insurance coverage for Permanent Full -Time Employees is <br />provided and paid for by the City. <br />Add the following sections: <br />Life Insurance Coverage <br />Permanent Full -Time Employees are required to purchase standard life <br />insurance coverage as provided by the City's insurance carrier. The cost <br />of this coverage is deducted from pay checks on a bi- weekly basis, with <br />the City contributing $9 per month toward the cost of this insurance. <br />Additional coverage limits can be purchased by the employee at his /her <br />discretion. <br />Long -Term Disability Coverage <br />Permanent Full -Time Employees are required to purchase long -term <br />disability coverage as provided by the City's insurance carrier. The cost <br />of this coverage is deducted from pay checks on a bi- weekly basis. <br />The following are Personnel Policy issues which require Council discussion and <br />direction: <br />Travel Allowance (Page 29) — <br />Discussion of $25.00 per day for meals. <br />Discussion of use of personal automobiles on City business and the City's <br />payment for damages or reimbursement of a deductible in the event of an <br />accident while conducting City business. <br />See survey information attached. <br />Page 37 <br />
