-3- <br />in with a dozer and mixed with dirt to eliminate voids. The <br />permittee shall maintain a five (5) foot separation distance <br />above the water table while filling. <br />F. Equipment. The permittee shall utilize the equipment of the <br />F.M. Frattalone Excavating and Grading Company, Inc. The <br />equipment shall be available for adequate operation of the site <br />and shall be provided with safety and noise control devices. <br />Fire fighting equipment shall be kept at the site during hours <br />of operation and arrangements made with the local fire protection <br />agency to acquire their services when needed. <br />G. End Use. The ultimate land use shall be for industrial shops and <br />yard space for equipment, parking, and material stockpiles. <br />H. Special Operating Requirements. Within one month from the effective <br />date of this permit, the permittee shall properly dispose of all <br />waste temporarily stockpiled on -site. The disposal shall be done in <br />accordance with the requirements outlined in this permit. Demolition <br />wastes shall be confined to as small an area as practicable, and <br />adequate dust control shall be provided. Open burning shall be <br />prohibited. The permittee shall provide emergency communications <br />by the use of two -way radio. <br />I. Operator. The operator shall be certified in accordance with the <br />MPCA's Operator Certification Rule 6 MCAR §§ 4.6088 - 4.6100. <br />J. Plans and Specifications. The permittee shall operate the disposal <br />system covered by this permit in accordance with the permit appli- <br />cation dated March 16, 1984 and plans received March 21, 1984 which <br />are attached hereto and incorporated herein. <br />Part III. CLOSURE AND POST- CLOSURE CARE <br />A. Closure. The permittee shall close the facility in accordance <br />with the cover requirements of Part II.D. and the plans received <br />March 21, 1984. <br />8. Plat. A detailed description, including a plat, shall be recorded <br />with the County Register of Deeds. The description shall include <br />waste locations, depth of fill, and other information of interest <br />to potential landowners. <br />C. Closure Certification. The permittee shall notify the Director <br />when closure is completed and shall submit a closure certification <br />for approval by the Director. The certification, signed by the <br />project engineer, shall certify, with any exceptions listed, that <br />the closure has been completed in accordance with the plans and <br />specifications and this permit. <br />PAGE -10 <br />
