disseminated to individuals or agencies subse- <br />quent to the collection of the data when specifi- <br />cally approved by the Commissioner as neces- <br />sary to carry out a function assigned by law. <br />3. All individuals asked by any employee <br />or agent of this city to supply private or confiden- <br />tial data concerning himself shall be informed of: <br />The purpose and intended use of the re- <br />quested data within the city; <br />`D Whether he may refuse or is legally re- <br />quired to supply the requested data; <br />c i Any known consequence arising from his <br />supplying or refusing to supply private or con- <br />fidential data; and <br />;d? The identity of other persons or entities <br />authorized by state or federal law to receive <br />the data. <br />4. Private data may be used by and dissemi- <br />nated to any person or agency if the subject(s) <br />of the data have given their informed consent. <br />When consent is given by the signing of an authori- <br />zation statement, such statement must be: (a) in <br />plain language; (b) dated; (c) specific in designating <br />the particular persons or agencies to whom the <br />data subject is authorizing to disclose information <br />about him /her; (d) specific as to the nature of the <br />information he /she is authorizing to be disclosed; <br />(e) specific as to the persons or agencies to whom <br />he /she is authorizing information to be disclosed; <br />(f) specific as to the purpose for which the infor- <br />mation may be used by any of the parties named <br />in clause (e), both at the time of the disclosure <br />and at any time in the future; (g) specific as to <br />its expiration date which should be within a rea- <br />sonable period of time, not to exceed one year <br />except in the case of authorizations given in <br />connection with applications for life insurance or <br />noncancelable or guaranteed renewal health <br />insurance and identified as such, two years after <br />the date of the policy. <br />PROCEDURES TO ASSURE THAT ALL DATA <br />ON INDIVIDUALS IS ACCURATE, <br />COMPLETE, AND CURRENT <br />Within the time and budgetary limitations <br />established by the city council, the Responsible <br />Authority and designees will review all data on <br />individuals in the possession of the city to deter- <br />mine if to the best of their knowledge, it is accur- <br />ate, complete, and current for the purposes for <br />which it was collected. All subjects of public and <br />private data, to the extent that this data is in <br />active use within the city, shall be notified when <br />the Responsible Authority has reason to believe <br />that the data on these individuals is not accurate, <br />complete, or current for the purposes for which <br />it was collected. It will be assumed that data <br />collected from data subjects is accurate, complete, <br />and current for the purposes for which it was <br />collected unless the data subject otherwise noti- <br />-10- <br />
