without first having applied for and received an amusement center license as hereinafter provided, Operation of four (4) or more amusement devices in a business establishment is prohibited, unless the area occupied by the amusement devices is licensed as an amusement center. 811.030. APPLICATION. Each applicant for an amusement center license shall apply to the City Clerk upon such forms as are provided by the City and shall answer all questions and provide all information required on said application. The application shall include a description of the area within which the amusement devices are to be located. 811.040. LICENSE FEE. Amusement center licenses shall be issued for one (1) year periods to qualified applicants. The annual license fee is One Hundred Dollars ($100.00), and said fee shall be paid at the time of making application. In the event said application is rejected, the full fee shall be refunded. 811.050. INSURANCE. The applicant for an amusement center license shall provide the City with a Certificate of Insurance evidencing liability coverage in the amount of $100,000/$300,000, with a provision that the City be held harmless. 811.060. REVOCATION. The Council may, after notice and hearing, revoke any license issued under this Chapter if the licensee is convicted of any violation of an ordinance, state law, or federal law relating to the use, sale or possession of non - intoxicating liquor or intoxicating liquor, or use or possession of gambling devices, or of any violation of Minnesota Statutes, Chapter 617, as amended, or of any violation of Chapter 1202 of this Code. The conviction of a person, while an occupant of the licensed premises, of a violation of any of the above described ordinances, state laws, or federal laws shall also be grounds for revocation of the license. The violation of any condition of the license shall be grounds for revocation cf the license. 811.070. CONDITIONS OF LICENSE. (A) No amusement center license shall be issued by the Council unless the applicant has demonstrated that the place to be licensed is properly ventilated and equipped with necessary toilets, washrooms, and similar public facilities, and that the operation of the place to be licensed will not be a public nuisance or detrimental to public safety, morals or welfare. (B) No amusement center shall be located within 800 feet of any public or private school.
