without first having applied for and received an amusement center <br />license as hereinafter provided, Operation of four (4) or more <br />amusement devices in a business establishment is prohibited, unless <br />the area occupied by the amusement devices is licensed as an <br />amusement center. <br />811.030. APPLICATION. Each applicant for an amusement center <br />license shall apply to the City Clerk upon such forms as are <br />provided by the City and shall answer all questions and provide <br />all information required on said application. The application <br />shall include a description of the area within which the <br />amusement devices are to be located. <br />811.040. LICENSE FEE. Amusement center licenses shall <br />be issued for one (1) year periods to qualified applicants. <br />The annual license fee is One Hundred Dollars ($100.00), and <br />said fee shall be paid at the time of making application. In <br />the event said application is rejected, the full fee shall be <br />refunded. <br />811.050. INSURANCE. The applicant for an amusement center <br />license shall provide the City with a Certificate of Insurance <br />evidencing liability coverage in the amount of $100,000/$300,000, <br />with a provision that the City be held harmless. <br />811.060. REVOCATION. The Council may, after notice and <br />hearing, revoke any license issued under this Chapter if the <br />licensee is convicted of any violation of an ordinance, state <br />law, or federal law relating to the use, sale or possession of <br />non - intoxicating liquor or intoxicating liquor, or use or <br />possession of gambling devices, or of any violation of Minnesota <br />Statutes, Chapter 617, as amended, or of any violation of Chapter <br />1202 of this Code. The conviction of a person, while an occupant <br />of the licensed premises, of a violation of any of the above <br />described ordinances, state laws, or federal laws shall also be <br />grounds for revocation of the license. The violation of any <br />condition of the license shall be grounds for revocation cf the <br />license. <br />811.070. CONDITIONS OF LICENSE. <br />(A) No amusement center license shall be issued by the <br />Council unless the applicant has demonstrated that <br />the place to be licensed is properly ventilated and <br />equipped with necessary toilets, washrooms, and <br />similar public facilities, and that the operation <br />of the place to be licensed will not be a public <br />nuisance or detrimental to public safety, morals <br />or welfare. <br />(B) No amusement center shall be located within 800 feet <br />of any public or private school. <br />
