Project Team & Key Personnel: The proposal shall include a listing of key personnel to be <br />assigned to this project and their respective roles. A resume of these individuals should also be <br />submitted. At a minimum, this listing is to include the principal, project manager, and field <br />superintendent to be assigned to this project. It is important to note any current assignments that will <br />overlap with this proposed project. <br />Past Performance: Provide a list of all CM jobs performed in the last five years with the <br />name of each client and their contact information as well as a brief description of the project, noting <br />any difficulties encountered. (Failure to list any contracts within the five year period that are <br />discovered by the owner shall disqualify a firm from further consideration.) The listing shall also <br />include the assigned project manager and job superintendent involved in each project. <br />Claims /Litigation: Disclose any litigation or arbitration cases your firm has been involved <br />with in the past five years. <br />Related Projects: A list of other contracts entered into by your team that are similar to this <br />project should be provided. Each should include the degree of involvement by your firm and the <br />personnel proposed to be assigned to this project. <br />Fee proposal: Describe the amount or method by which your firm will charge for its services. <br />Please break it down in to the following categories: <br />(1) Basic Services <br />(2) General Conditions (Field Office, mobilization, etc.) <br />(3) Reimbursable Expenses <br />(4) Supplemental Services that can be anticipated. 13e sure to describe what additional <br />professional services are included and excluded from this list. (i.e. testing or other <br />specialty services) <br />EVALUATION CRITERIA: <br />1. Experience and qualifications of key firm members assigned to this project. <br />2. Demonstrated ability to work effectively with the owners, architect, the owner's <br />representative, and other stakeholders. <br />3. Ability to meet the owner's needs for construction management. <br />4. Similar project experience and expertise of the firm and assigned staff. <br />5. Evaluations of past performance. <br />6. Demonstrated ability to provide the best constructability value and solutions for the owner's <br />budget. <br />7. Fee proposal. The owners will not automatically select the lowest cost service but <br />proposers are strongly encouraged to be competitive. <br />8. Understanding of the project as evidenced by the proposal. <br />4 <br />
