6. Smoking shall not be allowed in the immediate area around an open manhole. <br />' 7. Before entering the manhole, the steps shall be visually checked for their soundness. If there <br />is any question, a ladder should be used. <br />8. Whenever an employee must enter a manhole, another employee shall remain on the surface <br />1 to offer help if necessary. <br />9. All Minnesota State "confined space laws" shall apply where appropriate. <br />Trenches <br />1. Trenches five feet or deeper shall be securely cribbed, shored, or laid back to a stable slope <br />' and provided with a ladder or other reliable means of entrance and exit. Ladders shall be no <br />farther than 25 feet away from workers in the trench. <br />' 2. Shoring and trenching must be checked each day before work begins. <br />3. Trenches must be guarded by light, signs, fencing, etc., to prevent public access near the <br />work location. Alternate crossings for pedestrians shall be provided and marked when a <br />1 public way is disrupted by excavation work. <br />4. The ground opening shall be covered or guarded from public exposure overnight when work <br />1 exceeds one day in duration. <br />5. Employees shall not walk under or remain near power shovels and dump trucks during <br />loading and unloading operations. <br />Scaffolds <br />1. Scaffolding shall never be overloaded. Weight limits listed on scaffolding shall be rigidly <br />adhered to. Only the supplies necessary for one day's work shall be stored on the scaffold. <br />2. Scaffolding shall be set up only where the ground surface is firm and unlikely to settle or <br />1 shift. Scaffolding equipped with wheels shall be locked during use. <br />3. The area below the scaffold shall be barricaded in such a manner as to prohibit public access, <br />and signs shall be posted to advise the public of potential danger. <br />1 4. Scaffolds shall be inspected each day before use. <br />5. Scaffolds shall be equipped with a handrail, midrail, and toe boards. The floorboards shall be <br />1 interlocked and cleated. <br />WELDING AND CUTTING <br />Gas Welding and Cutting <br />1. Cylinder valves and hoses shall be inspected before use. The operator shall check to <br />determine leaks or weak spots in the lines. <br />1 2. The immediate area must be checked for flammable materials. Area should be cleaned <br />before operations and begin and after concluding operations. <br />3. If cylinder valves do not open through hand pressure, the supervisor should be notified and <br />1 the cylinder shall be tagged as defective. Wrenches and hammers shall not be used to open <br />valves. <br />4. Empty cylinders shall be securely closed and labeled "empty" and securely stored. <br />5. Cylinders shall be stored a safe distance from welding areas or other combustible processes. <br />1 12 <br />
