F. Pallet forks may only be used for their intended purpose. (Loading and unloading <br />equipment and materials.) <br />This policy is not designed to be complete or all encompassing. It is provided for use as a guide. <br />Employees must bring any other safety concerns not covered by this policy to the attention of <br />their supervisor. <br />LOCKOUT/TAG -OUT PROCEDURES <br />Purpose <br />The purpose of this procedure is to insure that machines or equipment are isolated from all <br />potentially hazardous energy, and are locked out or tagged out before employees perform any <br />servicing or maintenance activities where the unexpected energization, start-up, or release of <br />stored energy could cause injury. This includes activities during normal operation, which require <br />the removal of guards or safety devices unless the exposure is minor or routine and an integral <br />part of the operation. It is the City of Lino Lake's policy to tag -out and/or lockout all equipment <br />before and during repairs. <br />Procedure <br />1. It is the responsibility of the Department Head to insure that this procedure has been <br />reviewed by all maintenance personnel, Safety Committee members, department <br />heads /supervisors, and any other employees whose jobs may require them to maintain or <br />service energized equipment, or to operate this type of equipment as a part of their normal <br />job duties. <br />2. Appropriate employees shall be instructed in the safety significance of the lock - out/tag -out <br />procedure. Each new employee and other employees, whose work operations are or may be <br />in the area, shall be instructed in the purpose and the use of the lock - out/tag -out procedures. <br />3. Preparation for Lock -Out or Tag -Out <br />Make a survey to locate & identify all isolating devices to be certain which switches, valves, <br />or other energy isolating devices apply to the equipment to be locked or tagged out. More <br />than one energy source (electrical, mechanical, or others) may be involved. <br />4. Sequence for Lock -Out or Tag -Out Procedure <br />a. Notify all affected employees that a lockout or tag -out system is going to be utilized and <br />the reason therefore. The authorized employees shall know the type and magnitude of <br />energy that the machine or equipment utilizes, and shall understand the hazards thereof. <br />b. If the machine or equipment is operating, shut it down by the normal stopping procedure <br />(depress stop button, open toggle switch, etc.). <br />c. Operate the switch, valve, or other energy isolating device(s) so that the equipment is <br />isolated from its energy source(s). Stored energy (such as that in springs, elevated <br />machine member, rotating flywheels, hydraulic systems, and air, gas, steam or water <br />pressure, etc.) must be dissipated or restrained by methods such as repositioning, <br />blocking, bleeding down, etc. <br />d. Lockout the energy isolating devices with the assigned individual lock(s). The individual <br />that applied the lock is the only individual that should remove the lock. <br />24 <br />
