3.11 CERTIFICATE OF COMPLIANCE WITH MINNESOTA STATUTES 290.92 AND 290.97 <br />A. Upon completion of the Project and prior to Final Payment, the Contractor and all <br />subcontractors shall complete Minnesota Department of Revenue Revised Form IC -134. <br />This form, Affidavit for Obtaining Final Settlement of Contract with the State of Minnesota <br />and any of its Political or Governmental Subdivisions, is to be signed by a Department of <br />Revenue representative and forwarded to the Owner. Copies of this form can be <br />obtained by writing to the Minnesota Department of Revenue, Mail Station 6610, St. Paul, <br />MN 55146-6610 or by calling (651) 282-9999 or 1 (800) 657-3594.
