used. An employee may be requested to file a physician's <br />statement, signed by the physician and the employee, <br />indicating the nature of his or her illness. <br />. Upon separation of employment from the Employer, for any <br />reason other than discharge for just cause, the employee or <br />their designated beneficiary shall be paid one-half (1/2) <br />of all unused accumulated sick leave, provided that the <br />employee has ten (10) years of continuous service with the <br />employer at the time of separation. <br />E. Employees may contribute up to four (4) days of sick leave <br />annually to a health care savings plan (HCSP) after the <br />probationary period has been satisfactorily completed. The <br />contributions will be processed the first payroll in <br />December. <br />Employees that qualify for the sick leave benefit noted in <br />Section D will contribute one hundred percent of the <br />benefit to the HCSP upon separation. <br />ARTICLE 20: FUNERAL LEAVE <br />An employee receives three (3) days paid leave to attend to the <br />funeral of a member of the employee's immediate family. Funeral <br />leave must be taken on consecutive working days, one of which is <br />the day of death or the day of the funeral. If extended time is <br />necessary, such time may be granted by the City Administrator and <br />will be taken as sick, vacation, unpaid or a combination thereof. <br />ARTICLE 21: LEAVE OF ABSENCE WITHOUT PAY <br />Upon request, a leave of absence without pay may be granted by <br />the City Council for a period of up to ninety (90) days for <br />regular full-time employees. <br />ARTICLE 22: COURT DUTY <br />Any regular full-time or regular part-time employee who is <br />required to serve as a juror or as a witness in court regarding <br />City business shall be granted leave with pay while serving in <br />such capacity. Upon completion of jury duty, the employee shall <br />reimburse the City for the amount of jury duty pay, less the <br />amount received for traveling expenses. <br />ARTICLE 23: MILITARY LEAVE <br />19 <br />
