RESOLUTION NO. 8846 <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br />RESOLUTION APPROVING SEVERANCE FOR <br />PETE SZUREK, PARKS MAINTENANCE WORKER <br />WHEREAS, Pete Szurek has resigned as Parks Maintenance Worker for the City <br />of Mounds View; his last day of employment was October 13, 2017; and <br />WHEREAS, in accordance with the Mounds View Personnel Manual and Public <br />Works Collective Bargaining Agreement, Mr. Szurek is eligible for a cash payout of his <br />vacation hours and is eligible to receive 50% of his sick leave hours deposited into his <br />Retirement Health Savings Plan; and <br />WHEREAS, Mr. Szurek's balance of accrued and unused vacation is 141.16 hours <br />and his sick leave balance is 203.74 hours; and <br />WHEREAS, Mr. Szurek's rate of pay upon resignation was $28.29 per hour. <br />NOW, THEREFORE, BE IT RESOLVED that the Mounds View City Council <br />approves severance to Pete Szurek consistent with the Public Works Collective <br />Bargaining Agreement and the Mounds View Personnel Manual as follows: <br />Hours Hourly rate <br />Payout <br />Vacation Hours 141.16 $28.29 <br />$ 3,993.42 Cash Payout <br />50% of Sick Leave 101.87 $28.29 <br />$ 2,881.90 into RHSP <br />Total Severance: <br />$ 6,875.32 <br />Adopted this 23rd day of October, 2017. <br />Carol A.
