d <br />4 <br />. (1) The name and address of the applicant; <br />(2) A list of the equipment whielt he proposes to proposed to be used <br />in such collection; <br />(3) The place or places to wl.---ich the garbage and rubbish is to be hauled; <br />(4) The manner in which said garbage is to be disposed of; <br />(5) The portion of the city in which collections are to be made. <br />Section 6. The New Brighton City Code is amended by adding Sections 11-32 <br />through 11-36 as follows: <br />Sec. 11-32. Categories of Licenses. <br />The following categories of hauling activities must be separately <br />licensed: <br />(a) Hauling of recyclables. <br />Hauling of any non-recvcsables and vard wastes from residences <br />(c) Hauling of any non-ree -clables and yard wastes from commercial <br />establishments and/or m-ltiole dwellings. <br />Each application for a license a --id each license issued shall identify the type <br />of license and the specified vehicles to be used by the licensed hauler. <br />Sec. 11-33. Insurance Required. <br />Before a license is issue,. the applicant shall file with the Citv Clerk <br />evidence that the applicant ha; in effect public liability insurance fo?- the <br />hauler's business and for all vehicles in at least the sum of $500 0G0 for <br />injury for one person, $1,000,000 for the injury of two or more persons --n the <br />same accident, and $_100,000 fo, property damage. <br />Sec. 11-34. Rate Filing. <br />When the application is submitted, the applicant shall file wits the <br />City Clerk a schedule of proposed rates to be charged during the licensed <br />period. In the case of service to residences, a minimum of three rate levels <br />for regular service, priced on tZe basis of volume, will be provided very <br />licensee shall provide 14 days' -.rior written notification to the City and the <br />licensee's customers of any chain -re in rates to be implemented durirc the <br />license period. <br />Sec. 11-35. Requirements and Restrictions. <br />All licensees, and any oter haulers in the City, shall comply with all <br />of the following requirements arid restrictions: <br />4 <br />
