The latter choice <br /> must be approved by a supervisor, and the employee's absence must not create a work coverage 1111 <br /> problem for his/her department. <br /> The provisions of this policy shall apply to all employees who are not members of a union whose <br /> contract with the City may specifically set forth contrary provisions dealing with the subject <br /> matter of this personnel policy. <br /> Unpaid Leave of Absence <br /> The City of Mounds View may grant an unpaid leave of absence to an employee upon written <br /> request and approved. Prior to requesting leave, an employee must first exhaust <br /> paid leave(i.e.,vacation leave, accumulated compensatory time, flex time, or sick leave if <br /> conditions meet the conditions required in usage of sick leave policy.) <br /> An unpaid leave of absence requires advance written approval of the City Administrator. The <br /> unpaid leave of absence may be granted at the discretion of the City Administrator base r. <br /> consideration of the following factors: <br /> • Municipal service needs; <br /> • The employee's performance record and length of service; <br /> • The reason(s) for the request; <br /> • Any other relevant information; and <br /> • The Supervisor's and/or department head recommendation. <br /> • <br /> If granted,the unpaid leave of absence may not exceed ninety (90) continuous calci d :days. An <br /> extension beyond that period may be requested in writing and granted at the discretion of the <br /> City Administrator based on the above considerations. An unpaid leave on absence shall not <br /> exceed one (1) calendar year, except as noted below. <br /> Employee's serving in the armed forces or who have been disabled while performing job duties <br /> at the City of Mounds View, may have their unpaid leave of absence extended beyond one (1) <br /> calendar year, at the discretion of the City Administrator, based on the above considerations. <br /> Status of Benefits <br /> Benefits including sick, vacation,holidays or other forms of indirect compensation will not <br /> accrue during an unpaid leave of absence. Step increases and probationary periods will be <br /> extended by the length of the unpaid leave. This anniversary date will be used for all future pay <br /> increases. The employees hire date will not change because of such leave. <br /> Insurance Continuation <br /> The employee may continue to be covered by group health, dental and life insurance,but will be <br /> responsible for paying one hundred percent(100%) of the premium costs. During the first <br /> calendar month the employee will be responsible for paying the pro-rated portion of their <br /> monthly insurance premiums for the days they are on unpaid leave. <br /> 4110 <br /> The employee must return to work the first day following the end of the leave and must notify <br />
