RESOLUTION 8752 <br /> <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br /> <br />ACCEPT QUOTES AND AUTHORIZE CONTRACT FOR STORM SEWER REPAIRS ON <br />COUNTY ROAD H2 <br /> <br />WHEREAS, the storm sewer structure on County Road H2 has deteriorated and has <br />partially crushed lines causing the road surface to settle; and <br /> <br />WHEREAS, two quotes for repair were requested and received to repair the storm sewer <br />structure and lines; and <br /> <br />WHEREAS, Dave Perkins Contracting, Inc. had the lowest quote of $30,946.00 to <br />complete the repairs; and <br /> <br />WHEREAS, there is currently sufficient funding to repair this structure and lines in the <br />Storm Water System Maintenance Budget (745-4415-5150). <br /> <br />NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Mounds View, <br />Ramsey County, Minnesota as follows: <br /> <br />1. The bid proposal for $30,946.00 submitted by Dave Perkins Contracting, Inc. of <br />Nowthen, Minnesota for storm sewer repairs on County Road H2 is hereby approved. <br />2. City Staff is authorized to prepare a construction contract agreement between the City <br />of Mounds View and Dave Perkins Contracting, Inc. for $30,946.00. <br />3. The Director of Public Works is authorized to make approvals related to additional <br />work resulting from unknown conditions discovered during construction of the project <br />up to a combined total of $3,000 (10% contingency) on behalf of the City Council. <br />4. The Mayor and City Administrator are hereby authorized and directed, for and on <br />behalf of the City, to execute and enter into a construction contract agreement with <br />Dave Perkins Contracting, Inc. <br />5. Funding for this project will come from the Storm Water System Maintenance Budget <br />(745-4415-5150). <br />Adopted this 8th day of May 2017. <br /> <br /> ____________________________________ <br /> Carol A.
