Fiskars Inc. <br /> Exempt Benefits <br /> • GroupHealth h Plan <br /> You are eligible to enroll in Fiskars' Group Health Plan the first of the month following 30 <br /> days of service. This plan provides medical care for you and your eligible dependents. <br /> Premiums are entirely paid by Fiskars. <br /> Group Dental Plan <br /> You are eligible to enroll in Fiskars'Dental Plan the first of the month following 30 days of <br /> service. This Plan provides routine/preventative care for you and your family up to$1,000.00 <br /> per year/per participant. Single dental coverage is currently $3.00 per month and family <br /> coverage is currently $25.00 per month. <br /> Salary Continuation Plan <br /> Fiskars provides a Salary Continuation Plan(LTD)which covers you if you become disabled or <br /> ill and cannot work. This plan provides for up to six months of earnings at 100%of base salary. <br /> _• After that, insurance provides 60% of your salary until the age of 65. Premiums are entirely paid <br /> by Fiskars. <br /> Group Term Life insurance Plan <br /> You are covered by Group Term Life Insurance up to two times your base salary, plus an equal <br /> amount of Accidental Death and Dismemberment coverage. Premiums are entirely paid <br /> by Fiskars. <br /> Travel Life Insurance <br /> Travel Life Insurance covers you in the event that a loss of Iife occurs while you are traveling <br /> on company business. This plan provides an additional $250,000.00 in coverage. Premiums are <br /> entirely paid by Fiskars. <br /> Vacation and Holidays <br /> You will earn ten days of vacation during the first year of your service and fifteen days <br /> 0 beginning the second year.
