1005.12 1005.12 <br /> <br /> <br />c. The registration statement shall be made on forms provided by the Building Inspection Department which <br />shall include the following information: <br /> <br />(1) Name and street address of owner of multiple dwelling. <br /> <br />(2) Name and street address of the owner, operator or agent actively managing said multiple dwelling <br />who shall be designated as the recipient of notices and personal service for purposes of subdivision 1005.16(2) <br />of this Chapter. Said operator or agent shall be the individual responsible on a daily basis in the absence of the <br />owner for maintaining, repairing, furnishing, renting, letting for lease and supervising the multiple dwelling. <br /> <br />(3) Name and street address of all other operators or agents actively managing said multiple dwelling. <br /> <br />(4) Name and street address of all partners if the registrant is a partnership. <br /> <br />(5) Name and street address of all officers of the corporation if the registrant is a corporation. <br /> <br />(6) Name and street address of the vendees if the multiple dwelling is being sold on a contract for <br />deed. <br /> <br />(7) Legal address of the multiple dwelling. <br /> <br />(8) Number of toilet and bath facilities that are shared by the occupants of two (2) or more units. <br /> <br />(9) Number and kinds of units within the multiple dwelling. <br /> <br />(10) Height of multiple dwelling in stories. <br /> <br />(11) Construction of the exterior of the building. <br /> <br />(12) Total floor area of the building. <br /> <br />(13) Total area provided on premises for off-street parking.
