• Employees will bmasked tmprovide medical documentation supporting the FK8LAqualified illness. <br />• The Donated Vacation Bank will be tracked bvthe Finance Department (PoynoU). <br />• Employees who receive donated vacation time may receive no more than 80 hours (two (2) <br />weeks) within a rolling 12 -month period. <br />w The Vacation Donation Policy does not |irn|t or extend the time available under the Family <br />Medical Leave Act (FyNLA). <br />• The City Administrator will have the discretion to review requests if the employee is deemed <br />unable to return bowork or returns b»work on m part-time basis. <br />• Under esimilar program, the IRS has ruled that these payments are tobmconsidered wages and <br />therefore taxable income to the recipient. As a reou|t, the payments will be included in the <br />recipients annual VV -2 Form. State and Federal income tax. FICA/Medicare tax, and <br />Supplemental Retirement contributions, depending on the a||Q|bii|tv of the recipient, will be <br />withheld bythe City atthe time ofpayment. The IRS has also ruled that the employee donating <br />the vacation time realized no income for those hours and incurs no tax-deductible expense or <br />loss, either upon surrender mfleave orpayment tothe Vacation Donation Bank. <br />• The City specifically retains the right to administer the policy in any manner it deems to be in the <br />beat interest of the City' including the right to ornand, to a|bar' to further ||nn|t, or to eliminate the <br />policy without notice. <br />
