RESOLUTION NO 9139 <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br />APPROVING SEVERANCE FOR VANESSA VAN ALSTINE <br />CABLE TELEVISION COORDINATOR <br />WHEREAS, Vanessa Van Alstine has resigned as Cable Television Coordinator <br />for the City of Mounds View, her last day of employment is July 11, 2019; and <br />WHEREAS, in accordance with the Mounds View Personnel Manual section <br />345(A), Ms. Van Alstine is eligible to receive 50% of her sick leave hours deposited into <br />her Retirement Health Savings Plan; and <br />WHEREAS, in accordance with the Mounds View Personnel Manual section <br />345(B) Ms. Van Alstine will receive her vacation compensation deposited into her <br />Retirement Health Savings Plan; and <br />NOW, THEREFORE, BE IT RESOLVED that the Mounds View City Council <br />approves severance to Vanessa Van Alstine consistent with the Mounds View <br />Personnel Manual as follows: <br />Hours <br />Hourly rate <br />Payout <br />Vacation Hours <br />195.04 <br />$31.53 <br />$ 6,149.61 into RHSP <br />50% of Sick Leave <br />289.82 <br />$31.53 <br />$ 9,138.02 into RHSP <br />Total Severance: <br />$15,287.63 <br />Adopted this 22nd day of July, 2019. <br />Carol A.
