MOUNDS VIEW COMMUNITY CENTER <br />RESERVATION AND CANCELLATION POLICY <br />METiG3/SEMIIVARS <br />A deposit of 50% of the room rental fee is required to reserve a room. The reservation is not guaranteed until 50% of <br />the room rental fee has been received by the Mounds View Community Center. <br />The remainder of the room rental fee due 10 days after receiving your final bill. <br />FOOD AND BEVERAGE <br />A deposit of 50% of the food and beverage order must be paid 2 weeks prior to the event and the balance within 10 days <br />of receiving your final bill. <br />REFUNDS <br />No refund will be issued for cancellations occurring less than 30 days prior to the rental date. <br />I have read the above reservation and cancellation policy and understand that my reservation is not guaranteed until the <br />Mounds View Community Center receives 50% of the room rental fee and food beverage order. In addition, I <br />understand that the remainder of the room rental fee and food and beverage order is due two weeks following the rental <br />date. <br />I agree that all activities undertaken by myself or organization as part of this rental agreement, is at my or the <br />organizations sole risk and that the City of Mounds View, its employees and agents shall not be liable for any claims, <br />demands, injuries, actions or causes of actions whatsoever to me, the organization of mine or its property arising out of <br />or connected with my or my organizations participation in the activities related to this rental. I forever discharge the City <br />of Mounds View, its employees and agents from all such claims, demands, injuries, damages, actions or acuse of action <br />whatsoever, and that I have read and will adhere to all guidelines governing this facility. <br />Signature of Applicant <br />Date <br />
