PHASE I - PROGRAM DEVELOPMENT <br /> OCTOBER 20 - NOVEMBER 18 , 1983 <br /> 1111 <br /> I . Survey and Assessment of Needs <br /> A. Survey the needs of the communities through mailings , <br /> participant (past and present) interviews , random <br /> phone interviews and public "person-on-the-street" <br /> interviews . <br /> B. Assess survey results and provide written <br /> documentation of results and recommendations. <br /> II . Survey and Assessment of Facilities <br /> A. Survey available facilities and their costs within the <br /> communities to include but not be limited to churches , <br /> private schools , public buildings, private business, <br /> etc . <br /> B. Assess survey results and provide written <br /> documentation of results and recommendations. <br /> III . Evaluate current wellness programs available to residents <br /> to define areas which are either sufficient or deficient . <br /> IV. Establish goals and objectives for a Healthy Lifestyle <br /> • Program for the Cities of Mounds View and New Brighton <br /> based on the information gathered in I through III . <br /> V. Develop a comprehensive health, fitness, and lifestyle <br /> program( s) for the communities ' residents to meet the <br /> established goals and objectives , within a budget of <br /> VI . Apply for reimbursement grants through Group Health, Inc . <br /> other agencies. <br /> VII . Develop a comprehensive promotion 'and marketing plan as <br /> outlined in Item I-B. <br /> VIII . Seek out and apply for local funding through foundations , <br /> endowments , private corporations , etc . <br /> Ms. Linder will receive access to the support services of office <br /> space , files , phones and clerical services. <br /> In return for the aforementioned services ( I-VIII ) from Ms. <br /> Linder , the Cities will jointly provide reimbursement in the sum <br /> of $750 . 00 to be paid in two equal increments of $375 . 00 per pay <br /> period. In addition , she will receive up to $50 . 00/month for <br /> mileage and car allowance . <br /> • At the conclusion of Phase I , the Cities and Ms . Linder will <br /> evaluate the performance and development of the program and at <br /> this time can make adjustments where warranted. <br />
