PHASE I - PROGRAM DEVELOPMENT OCTOBER 20 - NOVEMBER 18 , 1983 1111 I . Survey and Assessment of Needs A. Survey the needs of the communities through mailings , participant (past and present) interviews , random phone interviews and public "person-on-the-street" interviews . B. Assess survey results and provide written documentation of results and recommendations. II . Survey and Assessment of Facilities A. Survey available facilities and their costs within the communities to include but not be limited to churches , private schools , public buildings, private business, etc . B. Assess survey results and provide written documentation of results and recommendations. III . Evaluate current wellness programs available to residents to define areas which are either sufficient or deficient . IV. Establish goals and objectives for a Healthy Lifestyle • Program for the Cities of Mounds View and New Brighton based on the information gathered in I through III . V. Develop a comprehensive health, fitness, and lifestyle program( s) for the communities ' residents to meet the established goals and objectives , within a budget of VI . Apply for reimbursement grants through Group Health, Inc . other agencies. VII . Develop a comprehensive promotion 'and marketing plan as outlined in Item I-B. VIII . Seek out and apply for local funding through foundations , endowments , private corporations , etc . Ms. Linder will receive access to the support services of office space , files , phones and clerical services. In return for the aforementioned services ( I-VIII ) from Ms. Linder , the Cities will jointly provide reimbursement in the sum of $750 . 00 to be paid in two equal increments of $375 . 00 per pay period. In addition , she will receive up to $50 . 00/month for mileage and car allowance . • At the conclusion of Phase I , the Cities and Ms . Linder will evaluate the performance and development of the program and at this time can make adjustments where warranted.
