ATTACHMENT A <br />PROPOSAL FORMS <br />Instructions: Please use this Word document to enter your responses to the RFP questions. When you are <br />done, please print and sign the final forms for submittal with your proposal. If multiple entities submit a <br />single proposal, Forms B, C, and D below must be submitted for each entity. <br />You may also print out this document, fill in your numeric answers by hand, attach additional sheets for <br />typed narrative answers, and then sign the forms. All forms completed manually must be clear and legible. <br />All forms must be completed and submitted for the proposal to be deemed responsive. The Proposer must <br />indicate clearly on the form(s) if any information is not applicable.
