The specific contract that we would be offering at <br />this time would include the following: <br />1. An annual salary of $19,000. <br />2. A full fringe benefit package as provided by the City of <br />Mounds Viaw including single Group Health coverage plus annu- <br />al dental coverage at no additional cost. Should you desire <br />famiiy coverage, it would cost an additional $23.35 per month <br />plus $29.30 for full family dental coverage. <br />3. Ten paid vacation days per year plus eleven paid holidays per <br />year. <br />4. Twelve paid sick days accrued at the rate of 4 hours per pay <br />period. <br />5. A paid life insurance policy in the amount of $5,000. <br />6. You are required to enroll in PERA, (Public Employee Retire- <br />ment Association) with 4 percent being deducted from each <br />payroll check and the City than matching it with an addition- <br />al 4-1/4 percent. I have included information regarding PERA <br />for your personal edification. <br />
