-2— <br />The following, are proposed timelines: <br />Request for Proposal due to Commissioner March 1, 1986 <br />Approval of proposals by Commissioner March 15, 1986 <br />Time interval proposed to cover April 1, 1986" <br />to June 30, 1987 <br />First persons receiving services June 1, 1986 <br />V. COMUNITY—INTEGRATED SERVICES PROPOSAL CRITERIA <br />A. The proposal shall be limited to serve regional center residents whose,;>�. <br />county of finnncir.l responsibility is one cf the following:', <br />1. The same county that the regional center is located in; or <br />2. A county within one hour's driving, time (50 miles) from the <br />regional. center. <br />R. An individual shall be eligible to receive pilot project services If <br />the individual meets the following criteria: <br />1. The individual is eligible for medical assistance under Minnesota <br />,...�y <br />Statutes, chapter 2568; and, <br />2. The Individual is a resident of a regional center;* and, <br />3. The individual is a person with mental retardation according to <br />!�'1 <br />the definition In part 9525.0015 jEmergencyl, subpart 22 and has <br />at least one of the following,-yaracteristics:f�( <br />a. Severe maladaptive behavior as listed in unit 1, 2, or 3; <br />(1) Self —injurious behavior which is a clear danger to the <br />client such as Ingesting inedibles; removi.ng major items <br />of clothing,; striking, biting, or scratching self; <br />moving into dangerous situations which clearly threaten <br />or endanger the cltent'n life, sensory abilities, limb <br />mobility, brain functioning„ physical appearance, or <br />other major physical functions; or <br />(2) Aggressive behaviors which are a clear dnnger to others <br />such as striking, scratching, or biting others; throwing -' <br />heavy objects at others; attempting inappropriate sexual <br />activity with others; or pushing or placing others into <br />dangerous situations which clearly threaten or endanger <br />their life, sensory abilities, limb mobility, brain <br />functioning, sexual integrity, physical appearance, or <br />other major physical functions; or <br />(3) Destructive behaviors which result to extensive property <br />damage; <br />*In addition to each project serving 20 residents from regional cen— <br />ters, Lounttes may work with these pro!ects to serve nddittonal <br />eligible persons from the community using waiver slots. <br />
