Medical conditions as listed In units (1) or (2); <br />(1) Degenerative diseases diagnosed by a physician as ter- <br />minal; or <br />(2) Short-term medical disabilities that can be treated <br />within the level of care the Minnesota Department of <br />Realch certifies an ICF-MR to provide, such as tempor- <br />ary immobility, intermittent catheterization, or post- <br />operative recuperation. <br />C. The county of financial responsibility will authorize the delivery of <br />services as defined 1n Minnesota Rules, parts 9525.0015 to 9545.0145 <br />[Energeccyl. Statutory requirements must be met prior to a resident's <br />movement out of a regional center. <br />D. People shall receive services in their county of financial responsi- <br />bility. It is the intent of this proposal to move persons currently <br />living in regional centers back to their home communities. While some <br />;.eople mny no longer have close family or community ties in their home <br />communities, moving them home will provide for the renewal of old rela- <br />tionships and/or the opportunity to make new ones. Variances will be <br />considered if adequate justification is made. <br />E. The pilot project will start 1.:oviding services to individuals by <br />June 1, 1986. At least two persons will begin receiving services by <br />this date. _ <br />F. Services provided shall be tailor-made to meet the individuel's needs. <br />One person may require a relatively unstructured environment, while <br />another person may require 24-hour supervision teamed by professional <br />residential staff. (See the attached Core Cluster Delivery System <br />Nanuhl for nervice examples.) <br />C. No mnre than four persons shall reside in the same house (single family <br />living unit). Preference will be given to proposals that use existing„ <br />ordlna'rq housing. No haysinq nhnll be adjacent to the regional center <br />grounds. (Adjacent means "next. to" or "across the street from.") <br />Housing shall be rented or leased. Rousing, used for services must meet <br />location and zoning requirements in accordance with Minnesota Statutes, <br />section 245.812. In order to provide individual, integrated services <br />to individuals and to avoid excessive grouping of persons with mental <br />retardation, it is Important to provide services in small home -like <br />settings. <br />If. There will be separate programs for children and adults. Permanency <br />planning requirements will apply to children programs. (See attached <br />Permanency Planning Requirements.) <br />
