:c <br /> (d) Provide a certificate of insurance showing proof of general <br /> liability insurance and professional liability insurance with a <br /> company authorized to do business in the State of Minnesota; <br /> (e) Keep the City apprised of current personal mailing address and <br /> telephone number; and <br /> (f) Agree to adhere to a code of ethics in performance of housing <br /> evaluation duties. <br /> Subd. 2. Compliance to Housing Evaluation Pro rg_am. Each evaluator is <br /> required to comply with the following procedure for preparing and filing <br /> housing evaluation reports: <br /> (a) The evaluation report must be either typewritten or legibly <br /> printed in ink on forms furnished by the City. The report shall <br /> indicate whether the condition meets the City of St. Anthony <br /> housing standards, is below the City of St. Anthony housing <br /> standards or is deemed to be hazardous at the time and date of the <br /> evaluation. <br /> (b) The evaluator shall submit the original housing evaluation report <br /> to the seller. A duplicate shall be filed with the City within 10 <br /> calendar days after the evaluation has been made. <br /> (c) A filing fee (?) shall be required with each evaluation report filed <br /> with the City. <br /> Section 1306.7 Correction of Immediate Hazards. The following items, when <br /> determined by the evaluator to be unsafe, shall be identified as immediate hazards in the <br /> evaluation report: <br /> Subd. 1. Heating Systems: <br /> (a) Burned out or rusted heat exchangers; <br /> (b) Burned out, rusted or plugged flues; <br /> (c) No vent; <br /> (d) Connection with unsafe gas supplies; or <br /> (e) Incapacity to adequately heat the living space. <br />
