illness of the employee' s child (an individual under 18 years <br /> of age or an individual under age 20 who is still attending <br /> secondary school) , for such reasonable periods as the <br /> employee's attendance with the child may be necessary, on the <br /> same terms the employee is able to use sick leave for the <br /> employee's own illness. <br /> Subd. 3 . Procedure. To be eligible for sick leave with pay, <br /> an employee shall (a) report as soon as possible to his or her <br /> Department Head the reason for his or her absence; (b) keep <br /> his or her Department Head informed of his or her condition; <br /> (c) submit a statement from a medical practitioner upon the <br /> request of the employer. <br /> Subd. 4 . Penalty. Employees using or claiming sick leave for <br /> a purpose not authorized by Subd. 2 shall be subject to <br /> disciplinary action up to and including discharge. <br /> Subd. 5. Accrual During Leave. Employees using accrued sick <br /> leave shall be considered to be working for the purpose of <br /> accumulating additional sick leave. <br /> Subd. 6. Worker's Compensation Benefits. Employees are <br /> covered by the Worker's Compensation laws of the state. In a <br /> disabling injury, workers are eligible for regular sick leave <br /> with pay benefits. However, any worker's compensation payment <br /> for lost wages is to be applied towards any sick leave <br /> compensation benefits taken. <br /> Subd. 7. An employee who has been unable to work because of <br /> illness or accident may be required, before being permitted to <br /> return to work, to provide medical evidence that the employee <br /> is again able to perform all significant duties of the job in <br /> a competent manner and without hazard to themselves or others. <br /> Subd. 8. Severance Pay. A severance pay policy is <br /> established subject to the followingrules and regulations: <br /> (a) Severance payments will be made to full-time <br /> employees only with at least one year of service who are <br /> permanently disabled, die, retire, i.e. , who are eligible <br /> for and apply for a pension, or who are laid off. <br /> (b) The employee must be in good standing and give <br /> proper notice of termination in the case of resignation. <br /> (c) Qualifying employees shall receive one-half of their <br /> accumulated sick leave up to a maximum of 45 days. <br /> (d) The payment for severance pay shall be made within <br /> 60 days after resignation. <br /> 3-7 <br />
