AMENDMENT FORM. <br /> STATE OF MINNESOTA E OWL= <br /> Minnesota Pollution Control Agency MAY 0 4 ','� AMMt1l' <br /> $20,240 <br /> iY! i�f-�, Ground Fate: <br /> City of St. Anthony C: Solid Waste Div. <br /> 3301 Silver Lake Road <br /> St. Anthony, Minnesota 55418 <br /> AmEmm Tr No. 1 TO CONTRA= NO. 32300-19754 <br /> WHEREAS, the State of Minnesota, Minnesota Pollution Control Agency has a <br /> contract identified as Contract No. 32300-19754 with the City of St. Anthony to <br /> rehabilitate city well #3, obtain pipeline easements and provide operation and <br /> maintenance for the Granular Activated Carbon Water Filtration Plant, and <br /> WHEREAS, the State and the City have agreed that additional tasks are necessary <br /> for the satisfactory completion of the contract; and <br /> WHEREAS, paragraph 4.6, Task 1 Rehabilitation of City Well 3, provides that: <br /> The City shall provide the state with an evaluation and justification for extent <br /> of rehabilitation required for long-tern performance of Well 3 and a work plan. <br /> Upon approval of the Work Plan by the Project Manager, the City shall <br /> subcontract the rehabilitation of City Well 3 at a cost not to exceed the <br /> • following: <br /> City labor and reimbusable costs $4,600 <br /> Subcontractor <br /> Normal Repair 21,360 <br /> New Motor and Controls (potential) 14,100 <br /> Bowl Replacement (potential) 10,600 <br /> Total $50,660 <br /> Well 3 shall be in sufficient operational condition at the completion of the <br /> work to begin normal pumping for the city water supply and treatment on <br /> long-tern basis. This task must- be completed by June 30, 1990; and, <br /> WHEREAS, paragraph 6.5 provides that: <br /> The contract dollar amount for this contract for Task 1 and 2 as described in <br /> Part 4.6 of this contract shall not exceed fifty three thousand six hundred <br /> sixty dollars ($53,660) . <br /> WHEREAS, the work plan submitted by the City has indicated that repairs to well <br /> #3 will expand the scope of work to include a ventilation system, an increase in <br /> motor size and its related reduced voltage starter and additional service <br /> capacity, necessitated by the new granular activated carbon facilities, and <br /> WHEREAS, the expanded scope of work has increased costs for the repair Well U. <br /> • NOW THEREFORE IT IS AGREED BY AND BETWEEN THE PARTIES HERETO: <br />
