• , -2- <br /> Consent Form, which is attached hereto and marked Exhibit A, prior <br /> to being vaccinated. <br /> 2. Bloomington shall administer three ( 3) separate vaccines <br /> for each St. Anthony employee. <br /> 3 . St. Anthony shall pay <br /> Bloomington the sum of One Hundred and Forty Dollars ($140. 00) for <br /> each employee consenting to be vaccinated. On or before a date to <br /> be determined by Bloomington, St. Anthony <br /> shall remit to Bloomington the total amount of compensation from <br /> all employees of St. Anthony who have consented to <br /> be vaccinated. It is understood and agreed that such compensation <br /> from all employees of St. Anthony who have <br /> consented must be paid prior to Bloomington dispensing the vaccine. <br /> 4 . Bloomington will conduct the vaccination program at ten <br /> to fifteen ( 10-15) separate clinics . Bloomington shall inform <br /> St. Anthony of the location of each clinic and <br /> provide a schedule for administration of the vaccine. <br /> 5. Bloomington shall provide to St. Anthony <br /> a report confirming that the consenting employees of St. Anthony <br /> received the Hepatitis B vaccine on three separate occasions . <br /> 6. It shall be the responsibility of St. Anthony <br /> to insure that its employees requesting immunization utilize the <br /> available clinics at the designated times . Bloomington shall not <br /> refund any payments to St. Anthony for employees <br /> who are not vaccinated because they failed to appear at a clinic <br /> during the times when the vaccine was being administered. <br /> • <br />
