SRTS <br />Information Needed for Planning Grant Application <br />Information Needed from the School District /Individual School <br />1. Data in the attached planning application for each school that is to be evaluated — fill in the box <br />— self - explanatory. <br />2. Names of Safe Routes To School Team Members— Members should include the following from <br />each school: <br />a. Principal <br />b. School or district nurse /health official <br />c. Transportation or facilities person <br />d. Teacher <br />e. Parents(s) <br />f. Student(s) <br />Names of parents and students could be provided if funding is secured. A commitment to <br />include all of the above should be included in a resolution. <br />3. Does the school have or participate in any of the following? <br />a. An existing city, county or district -wide SRTS program <br />b. Walking and or bicycling events, activities, training and clubs. If yes, list. <br />c. Already completed a walkability or bikeability audit <br />d. Have any school or district policies that prohibit walking /biking to school. If yes, explain. <br />4. Does the school have (or is it affected by) any of the following site planning policies? <br />a. Policies minimizing school size and /or promoting compact, central school locations <br />b. Collaborative school planning between the school district and local government <br />c. Policies that encourage or reward reuse /rehab of existing buildings <br />5. Does the school have (or is it affected by) any of the following health policies or programs? <br />a. School /district wellness policy promoting walking /biking to school <br />b. School health committee that oversees health and safety policies or programs <br />c. Active living, healthy communities or neighborhood health policies or programs <br />6. What is the school walk zone? Does it differ by grade? If yes, please list what the zone is. <br />7.
