Usage. Personal time off may be used as earned, subject to <br />approval by the Department Head and City Manager of the time at which it may be taken. <br />In order to allow coordination of -r%n-"OrN"r11 IRPATO <br />personal time off with family and <br />medical leave, employees seeking to take :ners-ena I lfmv ze personal time off may be <br />required to provide enough information about the purpose of the time off to allow a <br />determination of whether the time will also qualify for family and medical leave. If the <br />1 --,- personal time off also qualifies for family and medical leave, the employee <br />Y V <br />must take both leaves simultaneously. <br />For all full time employees hired after January 1, 2004, the -1 Y IZ7 <br />personal time off accrued shall not exceed 1,200 hours. <br />Subd. 3. Terminal Leave. Any employee leaving the municipal service in good standing <br />after giving proper notice of such termination of employment, will be compensated for <br />'-Pula personal time off accrued and unused to the date of separation according <br />to the schedule in Subd. 4-0 7 (b). <br />p -P q r% " p I T ga p"r r <br />Subd. 4. Waiver of Personal Time Off Prohibited. No employee is <br />clf-%"t-d 1Pkn:%r1a <br />permitted to waive perOWJ..L"-L.L%./"ve personal time off for the purpose of receiving double <br />compensation. <br />Subd. 5. Procedure. To be eligible for "r%*eOrN"n1 7e personal time off with pay, an <br />employee shall report as soon as possible to his or her Department Head the need to take <br />f-% inLo 1 r -L n X 7-,�A <br />UJL I"JL JLIC,� <br />PeTs , � personal time off and its estimated duration. <br />Subd. 6. Workers' Compensation. Employees are covered by workers' compensation <br />laws of the State. In the event an employee is disabled and is entitled to workers' <br />compensation, the employee will keep any workers' compensation payments received <br />and eligible to receive a bi-weekly pay check equal to 1/3 pay through the use of pe <br />keve personal time off benefits. In addition, the employee will be entitled to earn 1/3 of <br />the amount of <br />leave personal time off pay they would otherwise be entitled to <br />during an absence from their employment.
