City Council meeting of November 15, 2004 <br />Page 4 <br />APPROVE MEDICAL DIRECTION AGREEMENT W/REGIONS HOSPITAL FOR HFD <br />The Hugo Fire Department currently has a contract with Regions Hospital for medical direction, which <br />is a state statute requirement. Fire Chief Jim Compton and his officers have met with Regions Hospital <br />in order to finalize a five-year contract, which would provide medical direction coverage from 2005- <br />2010. City Attorney Dave Snyder has reviewed the agreement and found it to be satisfactory, with some <br />minor changes. Adoption of the Consent Agenda approved the Medical Direction Agreement with <br />Regions Hospital for medical direction oversight of the HFD for 2005-2010. <br />APPROVE APPOINTMENT OF BARBARA MAROHNIC AS NEW MEMBER OF THE <br />HISTORICAL COMMISSION <br />Upon the resignation of Historical Commission members Carrie Meyers and Paulette Carlson, City staff <br />has received an application from a resident for appointment to the Commission. Adoption of the <br />Consent Agenda appointed Barbara Marohnic as the newest member of the Hugo Historical <br />Commission. <br />APPROVE AWARD OF BID TO DEMOLISH WELLHOUSE #1 AND SEAL WATERMAIN <br />Staff received four bids for the demolition of wellhouse #1 and the sealing/plugging of the watermain <br />that extends to the wellhouse and water tower and opened them on Wednesday, November 10, 2004 at 3 <br />p.m. The low bidder was Peak Industrial Services at $7,155. Adoption of the Consent Agenda awarded <br />the bid to demolish wellhouse #1 and seal/plug the watenmain to Peak Industrial Services at $7,155. <br />APPROVE PAY REQUEST #7 FROM PARKOS CONSTRUCTION FOR WORK <br />COMPLETED ON THE NEW PUBLIC WORKS FACILITY <br />The City has received pay request #7 from Parkos Construction for work completed to date on the public <br />works facility. City architect Bob Russek from ESG Architects and PW Director Chris Petree have <br />reviewed the pay request and found it to be satisfactory. Adoption of the Consent Agenda approved pay <br />request #7 from Parkos Construction, in the amount of $362,136.01. <br />APPROVE PAY REQUEST FROM HULEGARRD CONSTRUCTION FOR WORK <br />COMPLETED ON THE ONEKA LAKE OUTLET PROJECT <br />The City has received pay request #1 from Hulegarrd Construction for work completed to date on the <br />Oneka Lake Outlet Project. Project Manager Pete Willenbring from WSB Engineers reviewed the pay <br />request and found it to be satisfactory. Council member Puleo requested this item be removed from the <br />Consent Agenda for discussion because of previous flooding problems. Water Resource Engineer Pete <br />Willenbring explained the grading and that more work would be done after cover had been established. <br />Puleo made motion, Petryk seconded, to approve payment to Hulegarrd Construction, in the amount of <br />$44,900, for work completed on the Oneka Lake Outlet Project. <br />All aye. Motion Carried. <br />
