49 <br /> <br /> <br />account. If an employee leaves employment with the City before retirement <br />and they desire to withdraw the amount they have contributed to PERA <br />(employees may not receive the portion paid by the City; it remains with <br />PERA), the employee may request such a refund, usually with interest. <br />Withdrawal forfeits any potential retirement payments which the <br />contributions may have earned for the employee. To be eligible for PERA <br />benefits, employees must be a member for a minimum of three years. <br /> <br /> Each year employees will receive correspondence from PERA detailing <br />contributions and other pertinent retirement information. Employees should <br />review the yearly information carefully to keep up-to-date on the benefits <br />accrued through the retirement fund. <br /> <br /> For additional information contact the City Administrator's Office. <br /> <br />56.2 Firefighter Retirement Provisions. <br /> <br /> Employees who are retiring from the Fire Department shall provide notice <br />to the Fire Chief at least 30 days prior to retirement. The Chief will notify <br />the City Council of the retirement. <br /> <br /> The retirement program is maintained by the Fire Relief Association. Hugo <br />Fire Department employees with ten (10) years of active service are eligible <br />to receive retirement benefits. The Fire Chief shall be responsible for <br />notifying the Relief Association in a timely manner upon notice of <br />retirement. <br /> <br />58.0 Resignations <br /> <br />Any employee wishing to leave the City’s service in good standing shall file with <br />the City Administrator, at least fourteen (14) days before leaving, a written <br />resignation stating the effective date of resignation. Failure to comply with this <br />procedure is cause for the City to deny the employee future employment with the <br />City and terminal leave benefits. Unauthorized absence from work for a period <br />of three working days may be considered as a resignation without such benefits, <br />with the exception of on-call employees. <br /> <br />59.0 Severance Pay <br /> <br />Upon resignation in good standing, retirement, death or disability of an <br />employee, the employee, or named beneficiary, shall be paid 100% of the <br />employee’s personal leave and compensatory time then accumulated, and the pay <br />shall be at the employee’s rate of pay on the last date of employment. <br /> <br />
