Report on 2023 Measures of Success and Additional Accomplishments <br />Measures of Success Report on Accomplishments <br />1.Finalize Agreement with the <br />City of Hugo <br />1 of 2 completed <br />1a – City of Hugo and Forest Lake <br />Historical Society Contract <br />complete. <br />Complete. The lease agreement between the City of Hugo, <br />the Forest Lake Historical Society (FLHS), and the Hopkins <br />Schoolhouse and Heritage Center (HSHC) was fully <br />executed on September 20, 2023. See attachment 1, p. 7. <br />1b – Contract transitioned to the <br />new nonprofit <br />Incomplete. The lease agreement between the City of <br />Hugo, FLHS, and HSHC, the agreement will transfer to <br />HSHC once the determination letter for HSHC’s 501c-3 <br />status is obtained. This measure could not be <br />accomplished due to the delay in submitting the IRS <br />application. See additional detail under measure 2d. <br />2.Form new nonprofit: Hopkins <br />Schoolhouse & Heritage <br />Center <br />4.5 of 8 Measures Complete <br />2a – New organization name <br />reserved through MN Secretary of <br />State’s Office <br />Complete. The HSHC name reservation was completed <br />on April 2, 2023. See Attachment 2, p. 34. <br />2b – Incorporated as a non-profit <br />organization with the MN <br />Secretary of State’s Office <br />Complete. On March 29, 2023, directors and officers were <br />appointed to the board of directors for the new non-profit, <br />and the articles of incorporation were adopted. The HSHC <br />application was submitted April 10, 2023, and a certificate <br />of incorporation from the MN Secretary of State was issued <br />on April 28, 2023. See attachment 3, p. 37. <br />2c – Application submitted for <br />Employer Identification Number <br />(EIN), and EIN Obtained <br />Complete. The HSHC received notice on February 12, <br />2024, with the EIN that the IRS assigned to HSHC. <br />See attachment 4, p. 43. <br />2d – IRS Form 1023 and <br />Application for Tax Exempt Status <br />submitted to IRS and <br />determination letter obtained. <br />Incomplete. The volunteer who had originally stepped <br />forward to assist with developing the application for tax <br />exempt status was ultimately not able to assist with this <br />activity in 2023. HSHC has 27 months from the time of <br />incorporation to complete this step. A new volunteer has <br />been identified to assist with completion of this step in <br />2024. <br />2e – Application submitted for <br />State of Minnesota Sales Tax <br />Exemption <br />Incomplete. This measure could not be accomplished due <br />to the delay in submitting the IRS application. See <br />additional detail under measure 2d. <br />2f – Registration as a charity with <br />the MN Attorney General’s Office <br />Charities Division <br />Incomplete. This measure could not be accomplished due <br />to the delay in submitting the IRS application. See <br />additional detail under measure 2d. <br />2g – Recruit minimum of 10 total <br />board members and 25 volunteers <br />Mostly Complete. HSHC was successful in recruiting two <br />new members to the board of directors, bringing the total <br />board member count to 9. During 2023, there were a total of <br />25 volunteers engaged in the project. See attachment 5, <br />p.46 . <br />Page 3 of 89
